Pathways to Transition Series

The Pathways to Transition Series is a free workshop for parents and professionals working with transition-age youth sponsored by Woodfords Behavioral Health Homes for Children's Transition Team. Offered in October, January and April, each two-hour Pathways to Transition workshop focuses on one area of transition planning and features a regional expert on the topic.

Woodfords Behavioral Health Homes for Children's Transition Program provides coordinated, comprehensive service and supports to provide continuity of care to the family during their child’s transition into adult living. The Transition Program focuses on vocational opportunities, guardianship, social security application, continued education, community supports, and eligibility determination for adult services.
