Signature Choices Form
You may use this form to request values that will appear on your email signature for certain fields: pronouns, credentials, and phone number.
All entries will be reviewed by the Development department for consistency with agency standards, then forwarded to the IT department to update your signature. Please allow up to one week for changes or new information to appear.
