Woodfords’ Adult Case Management Program is designed to provide a wide variety of supports to adults with intellectual and developmental disabilities and their families. Case managers link individuals and their families to resources and services in their own communities. Expeditions Community Supports Program

Expeditions, Woodfords’ Community Supports Program, provides community-based day services for adults with intellectual and developmental disabilities. Expeditions promotes active, healthy life styles by demonstrating the importance of healthy food choices, physical activity, new experiences, community involvement, learning new skills, and building relationships. Expeditions’ participants have the opportunity to engage in a wide variety of activities including sports, hiking, field trips, art and crafts, life skills practice, volunteering, and so much more.

Service Area

Woodfords Adult Case Management Program provides services to adults in York and Cumberland counties.

Eligibility & Referrals

To be eligible for adult case management services an individual must be 21 years or older, have a diagnosis of an intellectual or developmental disability, and qualify for MaineCare coverage. The Department of Health and Human Services (DHHS), Office of Aging and Disability Services (OADS) determines eligibility and provides referrals to case management agencies.

Expeditions Community Supports Program

Program

Expeditions, Woodfords’ Community Supports Program, provides community-based day services for adults with intellectual and developmental disabilities. Service Area

Expeditions is a year-round program operating Monday-Friday from 9 am -1 pm in two locations: the Westbrook Community Center, 426 Bridge Street, Westbrook, and at Woodfords’ Kennebunk Office at 5 Fletcher Street, Kennebunk.

Eligibility & Referrals

Participants must be 18 years old or older and qualify for Section 21 or 29 Waivers. Waivers are obtained through the Department of Health & Human Services (DHHS). Self-pay options are available if an individual does not have a Waiver.
